The Purpose of this Emergency Contact Data sheet is to notify the Emergency Management Coordinator of people within the community who have special needs to be considered during an emergency. While this database contains personal family/medical information that is considered confidential, the information contained in it is confidential and is for the use of the West Cape May Emergency Management Team or its designees only. If you have special needs that would require special attention during an emergency, please print this form, complete it and return it in a sealed envelope to Emergency Management Coordinator, Borough Hall, 732 Broadway, West Cape May, NJ 08204. Or you could drop it off in person to the Municipal Clerk’s Office, and it will be forwarded to the Coordinator.
